Hazel R. Levendoski, 72, of La Porte, passed away on Saturday, July 8th, 2023 at her home, surrounded by family and friends. 

   

Hazel was born on May 20th, 1951 to Howard and Leota (Cochran) Smalley. 

 

On June 8th, 1983 in La Porte, IN, she married Jon who survives.  Also surviving are daughters, Margaret Lummis, Cheryl Warner, Kimberly Matthews, and Carrie Howard, all of IN; 7 grandchildren, Michael, Ronnie, Jessie, Little Jon, Gabriel, Callie, and Corrine; siblings, Donald (Sue) Smalley of IN, Jerry Smalley of IN, Mercy (Bill) Adams of IN, Anne (Randy) Hensley of IN, and George Smalley of CT; dear friends, Rhody Peterson of La Porte and Bonnie (Mark) Smith of Fish Lake; as well as numerous nieces and nephews.

 

She was preceded in death by her parents and son, Eric Jon Levendoski.

 

Hazel was a laborer for many years at AeroMetals and Howmet.  She enjoyed camping, crocheting, taking drives, and her beloved dogs, Indy and Shadow.

 

A Celebration of Life Gathering will be held from 1:00 - 3:00 p.m. on Saturday, July 15th, 2023 at Cutler Funeral Home and Cremation Center.  Cremation will take place following the service.
