David Perkins, 54, of La Porte, Indiana, passed away Sunday June 13, 2021 at Northwest Health-La Porte.

 

 He was born August 23, 1966 in Toledo, Ohio, the son of Jim Cook and Olivia Cook.

 

David was an outdoorsman. He loved being outdoors enjoying fishing and hunting. He enjoyed working with wood and was a pretty good woodworker at that. He loved watching the WWE on TV with his favorite wrestler being "The Undertaker". But David's true passion in life was his family. He would spend time with them taking them fishing.

 

He is survived by Ex-Wife, Minnie Perkins of La Porte, IN; fiancé Sarah Feare; three sons, David Eugene Perkins, Jr., William Perkins, John Perkins; daughter, Mary Ann Perkins; three sisters, Vicki, Rose, Kathy; adopted brother, Jamie Cook; and adopted sister, Camey.

 

Arrangements are being entrusted to Haverstock Funeral Home and Cremation Services, 602 Maple Ave., La Porte, IN. The family has chosen cremation with a memorial service at a later date.
